Subscription fee:

Rs2500/- for old members
Rs 3000/- for new members

We will collect membership fees till 20th January 2018.

Any member who pays after 20th January will not be receive journal from FOGSI.

All the new members should provide copy of Aadhaar card, copies of MBBS , MD/DGO certificates ,APMC registration certificate and 2 passport size photos along with filled application forms

PG student should produce a letter from college HOD stating that you are postgraduate student

Application is for new members and for old members who need change of address or contact details.

Members please ensure that your contact details with FOGSI are correct as this is essential for verification of FOGSI membership.
